Most paperwork is for your child's safety and well-being.

Child's Enrollment Form

Completed at initial enrollment; reviewed annually to determine whether changes have occurred. Note that this is a 3 page form.

 

Child's Medical Form 

Completed on initial enrollment; children must have an annual well check.  A new form is completed and signed by the child's physician each year.

 

Child's Personal History 

Completed upon initital enrollment.  Generally only updated when family circumstances change or someone new is added to the authorized pick-up list.

Medication Administration

Completed for any medication, food supplement, modified diet, or topical product whether over the counter or prescription.

 

Sleep Position Waiver

Physician's authorization that allows an infant to be put to sleep on his or her stomach or side.  Infant's must be put to sleep on their back without this form.

 

Health Care Plan

Completed for children with any special health needs (special diet, asthma, allergies, epilepsy, diabetes, etc.)
